JourneyCare Hospice in Palos Heights, IL is seeking a Hospice Intake Coordinator to join our team. The role offers a 8:30AM-5PM schedule on weekdays, an on-call weekend rotation, and a training period at our Deerfield office.

Compensation ranges from $20 to $28 per hour, based on experience, with a generous benefits package including medical, dental, and vision, 401(k) with match, paid holidays, and tuition reimbursement.

Qualifications

• Be eighteen (18) years of age or older.
• At least one year experience in homecare, hospice, and/or healthcare field preferred.
• Ability to complete necessary documentation appropriately, legibly, and timely.
• Scheduling experience preferred.
• Ability to handle stressful and delicate situations.

Responsibilities

• Receive referrals by phone or in person for designated department.
• Provide explanation of benefits to patients, families, and community contacts.
• Assist with finding alternative services when the patient is not a candidate for hospice.
• Oversee scheduling of initial nursing assessment and admission.
• Maintain admission packets and ensure proper forms are available to admissions staff.
• Assist community outreach representatives with marketing activities to promote relations with referral sources.
• Assist with completion of admission paperwork as needed.
• Adhere to work schedule and maintain punctuality and attendance.
• Provide telephone and clerical support to other staff members as time allows.

• Interpersonal, verbal, and written communication skills necessary to complete job responsibilities
• Valid driver's license and proof of insurance are required

HOURS

Monday-Friday 8:30AM-5PM Including one summer and one winter holiday/annually.

On-Call Weekend Rotation Expectation

1x/month on-call weekend rotation in-office 8:30AM-5PM (Saturday and Sunday)

LOCATION

Palos Heights, IL.

Training Requirement

This position will be permanently based in our Palos Heights office. However, the initial training will take place at our Deerfield, IL office Monday-Friday for at least the first 4-6 weeks of employment depending on prior experience. Candidates must be able to reliably commute to the Deerfield location throughout the training period before transitioning to the Palos Heights location

Compensation
