Job summary

Horizons Hospice is seeking a dedicated Hospice Aide to provide in-home patient care under the supervision of a registered nurse. You will assist with transfers, ambulation, and daily activities while documenting observations and reporting changes to the RN.

The role requires a high school diploma or equivalent and CNA training/eligibility, CPR as applicable, and a valid driver’s license. Prior experience in hospice, hospital, or long-term care is preferred.

Qualifications

• Must be able to read, write, and verbally report clinical information to patients and staff.
• Demonstrates care for ill and elderly patients and families.
• Completion of required Aide Competency Evaluation and Skills Checklist.

Responsibilities

• Provide direct patient care under RN supervision according to Care Plan.
• Assist with safe transfers, ambulation, and daily activities.
• Report changes and document pertinent information to ensure continuity of care.
• Maintain infection control and a clean, safe home environment.
• Communicate respectfully with patients, families, and staff.
• Support nutrition and fluid intake for patients as directed.
