Reserve Care is seeking a Home Care Registered Nurse (RN) for full-time day shifts in the East Campus, Willoughby Hills, OH. You will provide prescribed medical treatment and personal care to terminally ill patients, coordinating with a transdisciplinary team to implement each patient’s Plan of Care.

You will assess needs, monitor symptoms, provide emotional support, and participate in care conferences, on-call rotations, and discharge planning while maintaining compliance with clinical

Competenze

• Licensed RN in Ohio.
• Hospice/home health experience preferred.
• Current CPR certification.
• Valid driver's license with good driving record.
• Hospice and Palliative Care Certification desired; must be obtained within first 2 years of hire.

Mansioni

• Provide intermittent skilled nursing services to terminally ill patients under physician direction.
• Assess physical, psychosocial, and spiritual needs of patient and family.
• Coordinate Plan of Care with the transdisciplinary team.
• Participate in on-call rotation and discharge planning.
• Maintain plans and reports related to hospice care.
