Job summary

Aurora is seeking a dedicated Home Health Aide to support patients in a hospice setting in Wisconsin. The role involves visiting patients at home, providing personal care per the plan created by the registered nurse, and supporting basic health monitoring.

Strong communication and reliability are essential. Typical hours are Monday through Friday, 8:00am to 4:30pm, with weekend rotation and occasional holidays to meet department needs.

Qualifications

• Excellent communication and listening skills are required.
• Must be listed in Wisconsin Nurse Assistant/Home Health Aide registry or eligible for testing.
• Experience in home health care or with elderly, convalescent, or disabled persons is preferred.

Responsibilities

• Makes home visits to patients and provides personal care per the plan developed by the nurse.
• Assists with bathing, dressing, skin care, and simple wound care.
• Records vital signs and input/output; documents health information.
• Assists with transfers, mobility, and positioning.
• Assists with meals and urine/sugar testing when appropriate.
• Maintains home environment cleanliness and safety related to care.
• Communicates with physicians and health care team and participates in quality improvement.
