Job summary

21st Century Health Care Consultants seeks a CNA/Home Health Aide to provide direct personal care in clients’ homes in the Aurora area. You will work under the supervision of a staff nurse and Nurse Supervisor, performing tasks such as personal care, hygiene assistance, and basic healthcare monitoring.

The role requires completion of approved CNA training and CPR certification, with ability to document visits weekly and coordinate with the RN for any changes in the plan of care.

Qualifications

• Must have CPR certification (current).
• Nurse aide training and competency evaluation program as specified by 42 CFR or state registries.
• DORA nurse aide certification or equivalent training.
• Criminal background check completed; in good standing with DORA.
• Clear ability to communicate in English (verbal and written).

Responsibilities

• Follow the plan of care to provide safe, competent home care.
• Assist with personal care, hygiene, and basic health maintenance.
• Support patient with ambulation and daily activities under RN supervision.
• Assist with basic housekeeping and meal preparation as instructed.
• Coordinate care with nurses, therapy personnel, and family; document visits weekly.
