Job summary

Advocate Aurora Hospice is seeking a Home Health Aide to deliver compassionate care in patients' homes in the Kenosha area. The role includes personal care, basic medical measurements, and strict documentation, under the supervision of an RN, with regular weekday hours, rotating weekends, and holiday coverage.

A state-certified nurse assistant and BLS are required, plus a valid driver's license for travel within the service area.

Qualifications

Must be certified as a nurse assistant in the state in which the teammate practices.
Nurse assistant in good standing with the State Nurse Aide Registry with eligibility to work in home health agencies and hospices.
Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 6 months.
A valid driver's license issued by the Division of Motor Vehicles.

Responsibilities

Provides personal care needs including feeding patient, laundry, light cleaning of patient room and washing.
Takes temperatures, pulse, respiration and blood pressure as directed by the RN.
Records patient's care documentation and maintains confidentiality per HIPAA and agency standards.
Demonstrates professionalism, flexibility in scheduling, and teamwork with colleagues and departments.
Adheres to regulatory requirements and completes annual in-service hours and OSHA requirements.
