Job summary

Bristol Hospice in Burr Ridge, IL seeks an experienced Certified Home Health Aide (CHHA) to support patients in their homes under RN supervision. You will perform personal care, monitor comfort, and maintain dignity and confidentiality while following the plan of care.

• Hospice or home health experience preferred.

Responsibilities

• Provide personal care including baths, grooming, and changing bed linen as assigned.
• Assist with meals and feeding as directed by the care plan.
• Monitor patient condition and report changes to the Case Manager.
• Maintain a clean, safe living environment and observe safety protocols.
• Document care provided and participate in plan-of-care updates.
• Support safe transfers, mobility, and comfort measures as instructed by the nurse.
