Patriot Healthcare seeks a compassionate Hospice Certified Nursing Assistant (CNA) to provide personal care to assigned patients in accordance with nursing assessments and care plans under supervision.

You will collaborate with the care team to implement the patient/family plan, assist with daily living activities, monitor comfort, and document care provided while maintaining a warm, respectful environment in a hospice setting.

Formación

• Participation in patient care activities as directed by nursing staff.

Responsabilidades

• Stewards the mission of Patriot Healthcare emphasizing dignity in terminal illness care.
• Participates with the IDG in developing and implementing patient/family plans of care.
• Performs tasks per Hospice policies and supervisor instructions.
• Maintain patient comfort and assist with daily living activities as requested.
• Document care provided and communicate with family or facility staff.
