Job summary

Patriot Healthcare is seeking a Hospice Certified Nursing Assistant (CNA) to provide patient personal care in accordance with nursing assessments and care plans, under nurse supervision. This role emphasizes compassionate, dignity-centered care within a Hospice setting in Myrtle Beach, SC.

The CNA will collaborate with the IDG to develop care plans, follow policies, document visits, and communicate with families.

Qualifications

Hospice CNA certification or license preferred.
Excellent communication with patients, families and supervisors.
Compassionate, patient-centered care with dignity.
Ability to work as part of a healthcare team.

Responsibilities

Provide personal care for assigned patients per nursing assessments and care plans.
Participate with the IDG in developing and implementing patient/family plan of care.
Follow Hospice policies and procedures as directed by supervisor.
Ensure patient comfort at all times and assist with activities of daily living.
Document tasks performed and any comments related to visits.
Communicate with family or facility staff to coordinate nursing services.
Create and maintain a warm, personal, calm environment for the patient.
Report concerns to supervisor and address any complaints or grievances.
Assist with special treatments under supervision as instructed.
