Job summary

Amedisys Home Health in Lancaster, PA is seeking a dedicated hospice aide for full-time days to support care at home. You will assist patients with daily living activities, meals, mobility, and basic health monitoring under the nurse's plan of care.

Responsibilities include personal care, safety-guided procedures, documentation, and teamwork with the care team to ensure comfort and dignity for patients in their homes.

Qualifications

• Must have PA CNA license with one year of experience.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Provides personal care services to patients.
• Assists with ambulation, transfers, positioning, and ROM.
• Plans and prepares nutritious meals and assists with feeding.
• Performs homemaking and environmental tasks as assigned.
• Observes and reports changes in patient status to the nurse.
• Follows written care instructions from the RN.
• Assists with prescribed exercises and basic urine tests.
• Performs other duties as assigned.
