Job summary

Amedisys Home Health is seeking a hospice aide to join our home health team in Pennsylvania. We offer full-time days with competitive pay and a focus on compassionate in-home care.

Qualifications include a PA CNA license, CPR, and a valid driver’s license. You will provide personal care, meal support, and assist with therapy-driven exercises as part of a patient-centered plan of care.

Qualifications

• High school diploma or equivalent.
• Current state CNA licensure/registration.
• CNA experience within last two years.
• Current CPR certification.
• Valid driver's license and insurance.

Responsibilities

• Provides personal care services to patients.
• Applies safety principles and proper body mechanics to the performance of personal and supportive care such as ambulation, transferring, ROM, positioning, and homemaking.
• Plans and prepares nutritious meals, assists with feeding, as assigned.
• Performs homemaking and other environmental services as assigned.
• Observes, reports, and documents changes in patient status.
• Follow written plan of care prepared by a registered nurse.
• Assists patients with prescribed exercises as instructed by nurse or therapist.
• Performs and records simple urine tests for sugar, acetone, and albumin.
• Other duties as assigned.
