Job summary

Providence at Home with Compassus seeks a Chaplain to provide spiritual support to patients and families in a hospice setting. You will join the Interdisciplinary Team to develop and implement care plans, counsel through death and bereavement, and coordinate with clergy as requested.

Qualifications

• High school diploma or GED; Bachelor's degree in Divinity or related field preferred.
• Minimum one year of spiritual/pastoral support experience.
• Completion of at least one unit of CPE preferred, if required by state.
• Commitment to Compassus Pillars of Success and equal opportunity employment.

Responsibilities

• Explains hospice services and Medicare benefits to patients and families; obtains required documents.
• Submits paperwork to facilities in a timely manner per policy.
• Articulates the regulatory framework governing spiritual care (federal/state).
• Participates as a member of the IDT; develops and implements care plan.
• Provides spiritual care to patient and family; supports involvement of community clergy.
• Collaborates with IDT to coordinate spiritual care while ensuring quality.
• Maintains comprehensive care plans reflecting problems, goals and interventions.
• Liaises with clerical community to interpret needs and recruit volunteers.
• Performs other duties as assigned.
