Job summary

Guardian Hospice is seeking a registered nurse to plan, organize and direct hospice care in home and community settings, leveraging community resources to meet patient and family needs. You will perform assessments, write and implement care plans, administer medications, educate families, act as case manager, and participate in on-call rotations.

Qualifications include NLN-accredited nursing program graduate, Oklahoma RN license, 2 years in public health/home/hospice preferred, CPR, and valid

Qualifications

• Graduate of National League for Nursing accredited school of nursing.
• Two years of experience, at least one in public health, home care, or hospice nursing preferred.
• Registered nurse with current licensure to practice in the state.
• CPR certification and valid driver’s license required.
• Self-directed and able to work with minimal supervision.

Responsibilities

• Completes initial and ongoing assessments of patient and family to determine hospice needs.
• Develops and revises the plan of care with nursing diagnosis and goals.
• Administers medications and treatments as prescribed and counsels patient/family.
• Acts as Case Manager coordinating patient care for assigned caseload.
• Provides education, documentation, and participates in interdisciplinary meetings.
• Participates in on-call duties and ensures equipment/services are available.
