Job summary

Sage Hospice Primary & Palliative Care in Arizona is seeking an RN to administer skilled nursing care for clients at home, coordinate care with the interdisciplinary team, patient and family, and manage overall coordination of services.

Qualifications include: current RN license, CPR certification, an accredited ADN/BSN, and at least two years’ RN experience in clinical care; hospice or home health preferred.

Qualifications

• RN license required.
• Two years’ RN experience in a clinical care setting; hospice or home health preferred.
• CPR certification required.
• Proficient computer skills; Microsoft Office experience preferred.

Responsibilities

• Deliver skilled nursing care in patients' homes.
• Coordinate care with the IDT, patient, family, and referring agency.
• Document progress notes and care plans.
• Initiate and revise care plans as needed.
• Provide patient and family education on disease process and care at home.
• Manage on-call responsibilities as assigned.
