Job summary

Sacred Heart Home Health invites a compassionate Registered Nurse to lead hospice care at patients' homes in the Tucson area. The role focuses on planning, organizing, and directing nursing services in community settings to support patients and families.

The RN will assess, plan, implement, and evaluate care, coordinating with the patient’s team to maximize comfort and quality of life at home.

Qualifications

• Must hold an active RN license.
• Experience in hospice or community health preferred.
• Ability to assess patients and families in home settings.
• Plan and direct nursing care using the nursing process.

Responsibilities

• Complete initial and ongoing patient/family assessments.
• Provide professional nursing care per physician’s plan of care.
• Develop and revise care plans with patient/family involvement.
• Initiate preventive and rehabilitative nursing procedures.
• Counsel patients and families on nursing and related needs.
• Provide health care instructions according to assessment and plan.
• Assist patients with activities of daily living and support self-sufficiency.
