Job summary

Affinity Health Management seeks an experienced registered nurse to direct hospice care within home and community settings in Connecticut. You will assess patients, plan and revise care, and collaborate with families and an interdisciplinary team to deliver compassionate palliative services.

The role emphasizes patient education, coordination of care, and adherence to professional nursing standards, with on-call duties and ongoing professional development.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse with license in good standing.
• One year of professional nursing experience in the last three years, or a BSN with two years' related experience.
• Management experience not required; may supervise hospice aides.
• Offer contingent upon references and criminal background checks.
• Excellent observation, verbal and written communication skills, problem solving and math.
• Must be able to lift and transfer patients; standing/walking duties may be extended.
• Must have a valid driver's license with insured vehicle.
• CPR certification required (hands-on verification preferred).

Responsibilities

• Complete initial and ongoing assessments of patients and families to determine hospice needs.
• Provide professional nursing care using all elements of the nursing process.
• Develop and revise care plans based on patient status and goals with patient/family involvement.
• Initiate and modify plans of care; document care plans and outcomes.
• Coordinate care with physician orders and interdisciplinary team.
• Counsel patients and families; provide health instructions and self-care guidance.
• Maintain safe patient environments and assist with activities of daily living as needed.
• Participate in on-call duties and ensure equipment and services are available.
