Job summary

Affinity Health Management in Washington is seeking a compassionate hospice aide to work under the supervision of a registered nurse. You will perform personal care tasks and document observations to promote patient comfort and quality of life per the plan of care.

Responsibilities include assisting with bathing, dressing, feeding, transfers and range of motion, providing emotional support, ensuring safety, and reporting changes to the RN Case Manager.

Qualifications

• Completion of a state approved certified nursing assistant course.
• Nursing Assistant Certified (NAC) certification.
• Current Healthcare Provider CPR certification.
• Current Food Handlers Permit.
• High school diploma or GED.
• Ability to read and follow written instructions.
• Experience in hospice or long-term care preferred.
• Basic computer skills to document in EMR.
• At least 18 years of age.
• Licensed driver with insured vehicle.

Responsibilities

• Perform personal care needs following patient assignments.
• Provide patient and family with supportive communication and confidentiality.
• Meet safety needs and use equipment properly (foot stools, side rails, O2).
• Assist with baths, dressing, transfers, and meals as assigned.
• Report changes to the RN Case Manager promptly.
• Document care provided in EMR and ensure accuracy.
