Job summary

Heart to Heart Hospice seeks a compassionate Hospice Aide (CNA) to provide direct patient care under RN supervision in a home-based or facility setting. You will help patients with transfers and ambulation and document care to maintain continuity of services.

The role requires an unencumbered CNA license, a high school diploma or GED, at least one year in hospice or nursing aide work, reliable transportation, and a willingness to adhere to infection control and privacy standards.

Qualifications

• High School Diploma preferred or GED.
• Unencumbered CNA license.
• At least 1 year experience as Hospice Aide or Nursing Assistant.
• Current driver's license and reliable transportation.
• EMR experience is a plus.

Responsibilities

• Provide direct patient care under RN direction and Plan of Care.
• Assist with safe transfers and ambulation per policy.
• Report changes and document care to ensure continuity of care.
• Follow infection prevention principles.
• Communicate respectfully with patients, families and staff.
• Submit documentation promptly and accurately.
• Comply with laws, Medicare regulations and policies.
