Provide direct patient care under the supervision of an RN, assist with activities of daily living, and help create a safe, comfortable home environment for patients. You will report changes to the supervisor, document care, follow infection control practices, and support families with compassionate communication.

Qualifikationen

• Current CNA/HHA certification and LTC certification.
• Valid driver's license and auto insurance.
• CPR certification required.
• HomeCare HomeBase experience preferred.

Aufgaben

• Provide direct patient care under RN direction.
• Assist with safe transfers and ambulation per policy.
• Report changes and document care rendered for continuity of care.
• Maintain infection control and provide a clean, safe environment.
• Promote respectful communication with patient, family and staff.
• Assist with proper nutrition and adequate fluid intake.
• Contribute to efficient operation and time management of the agency.
• Perform treatments and procedures per policy.
• Uphold agency philosophy and administrative policies.
