Qualifications

  • Successful completion of a state-established training program or licensure meeting S 484.36(a) standards, or equivalent competency program.
  • Good verbal and written communication and problem solving skills.
  • Valid driver’s license; reliable insured automobile for home visits.

Responsibilities

  • Performing personal care needs after a patient’s assignment and attending to requests promptly.
  • Meeting safety needs of patient and using equipment safely and properly.
  • Providing personal care including baths, back rubs, oral hygiene, shampoos and changing bed linens as assigned
  • Assisting in dressing and undressing patients as assigned
  • Assisting with patient meals as assigned
  • Providing proper care and observation of patient’s skin to prevent tissue breakdown
  • Reporting on patient’s condition and significant changes to the supervising nurse
  • Assisting in ambulation and exercise as instructed by the nurse or therapist
  • Offering and assisting with bedpans and urinals
  • Providing assistance with light laundry needs
  • Performing range of motion and simple procedures as ordered
  • Providing respite for patient’s family/caregiver on-site
  • Keeping patient’s living area clean and orderly
  • Adhering to documentation and care procedures and standards of conduct
  • Respecting patient/family/caregiver environment and patient’s personal needs
