Zusammenfassung

Infinity Hospice Care, part of the LCHH family, provides in-home hospice and related services across multiple states. The Hospice Aide will assist patients in their homes under RN supervision, ensuring safe environments and compassionate care.

Responsibilities include direct patient care, safe transfers, infection control, and timely documentation. Strong interpersonal skills and reliable transportation are essential for effective patient support and collaboration with the care team.

Qualifikationen

• High School Diploma preferred. Completion of nurse aide training/competency program is required or state licensure.
• Experience: at least one year as Hospice Aide or Nursing Assistant preferred.
• Must be able to read, write, and verbally report clinical information to patients, families, care givers, and hospice staff.

Aufgaben

• Provide direct patient care under RN guidance according to the Aide Plan of Care.
• Assist patients with transfers and ambulation per Hospice policy.
• Report changes and document care rendered to ensure continuity of care.
• Maintain infection control and provide a clean, safe home environment.
• Communicate respectfully with patients, families, and staff.
• Support nutrition and fluid intake for patients.
• Participate in on-call/after-hours coverage as assigned.
