Job summary

Infinity Hospice Care LLC is seeking a dedicated Home Health Aide to support patients in their homes, providing personal care, mobility assistance, feeding, and exercises as outlined in the care plan.

You will document care activities, communicate changes to the care team, and participate in team meetings, while adhering to infection control and licensing requirements. Some travel to patient residences is required.

Qualifications

• Ability to establish and maintain effective working relationships with staff and leadership.
• Shall be able to read, write and comprehend English.

Responsibilities

• Assist clients with tasks within the scope of a home health aide based on the Plan of Care (POC).
• Communicate effectively with administrative staff regarding client/family needs assessments, including reporting changes in client condition.
• Provide care as outlined in the Home Health Aide Care Plan and document cares provided accurately.
• Contribute to continuity of care and participate in Case conferences/Team Meetings.
• Apply infection control and universal precautions in client care activities.
• Participate in in-service programs and maintain/ improve competency; complete training as assigned.
• Meet mandatory continuing education requirements of the Agency/licensing board.
