Job summary

Affinity Health Management seeks a compassionate Hospice Aide to support patient comfort under RN supervision in Ohio. You will perform personal care, assist with activities of daily living, and document observations as part of the patient’s plan of care.

You will report changes to the RN Case Manager and help maintain safety, cleanliness, and effective communication with patients and families. A CNA certification, CPR, and a valid driver’s license are required.

Qualifications

• Completion of a state approved certified nursing assistant course or equivalent training.
• Current Healthcare Provider CPR certification (hands-on verification required).
• Covid vaccination as per facility guidelines.
• High school diploma or GED completed.
• Ability to read and follow written instructions and document care.
• Prior hospice/long-term care experience preferred.
• Basic computer skills to document in electronic medical records (EMR).
• Must be at least 18 years of age.
• Valid driver’s license and insured vehicle per organization policy.

Responsibilities

• Perform personal care needs and attend to patient requests promptly.
• Provide compassionate communication with patients and families.
• Ensure safety and proper use of equipment (bed rails, O2, etc.).
• Assist with bathing, grooming, oral care, and changing bed linens.
• Assist with dressing, feeding, and mobility as directed by RN.
• Observe and report pain or changes to the RN Case Manager.
• Assist with ambulation and range of motion as instructed.
• Support medication reminders as allowed by state rules.
• Keep patient’s living area clean and organized and document care.
• Adhere to care plans, policies, and professional conduct standards.
