Job summary

Affinity Hospice Management LLC in Des Moines is seeking a dedicated hospice aide to support patients under RN supervision. You will perform personal care, monitor comfort, and document observations in the patient’s plan of care to promote safety and quality.

Responsibilities include assisting with bathing, dressing, feeding, transfers, and basic home care tasks while reporting changes to the RN Case Manager.

Qualifications

• State-approved CNA course or equivalent training is required.
• NAC certification is required or preferred.
• CPR certification is required; hands-on verification preferred.
• Food handlers permit is required.
• High school diploma or GED required.
• Ability to follow written instructions and document care.
• Understanding hospice philosophy and terminally ill care.
• 1 year home health/hospice experience preferred.
• Basic EMR documentation skills.
• Must be at least 18 years old.
• Valid driver with insured vehicle.

Responsibilities

• Provide personal care and meet patient requests promptly under RN supervision.
• Communicate with patient and family with confidentiality and respect.
• Ensure safety and proper use of equipment during care.
• Assist with baths, back rubs, oral hygiene, and changing bed linens.
• Aid in dressing and undressing patients as assigned.
• Assist with feeding; avoid homemade foods prepared by staff.
• Monitor skin condition to prevent tissue breakdown.
• Report pain or changes to the RN Case Manager promptly.
• Assist in ambulation and prescribed exercises.
• Support with ROM, transfers, and positioning as directed.
• Assist with self-administered meds as allowed by state regs.
• Offer bedpans/urinals and light housekeeping tasks.
• Perform routine range of motion and simple procedures as trained.
• Provide respite when needed for patient/family/caregiver.
• Keep living areas clean and orderly per procedures.
• Adhere to documentation and care standards.
• Participate in quality improvement activities as required.
• Respect patient and family environment and personal needs.
• Report suspected abuse or neglect per Department guidelines.

Education

State approved Certified Nursing Assistant course or equivalent training

NAC

CPR certification

Food Handlers Permit

High school diploma or GED

Ability to read and follow written instructions and document care

Hospice philosophy understanding

1 year home health/hospice experience preferred

Basic computer skills for EMR documentation

At least 18 years of age

Licensed driver with insured vehicle
