Job summary

Affinity Hospice Management LLC is seeking a compassionate Registered Nurse to plan, organize and direct hospice care in patients' homes and communities in Des Moines, IA. You will perform comprehensive assessments and coordinate the care team.

You will educate patients and families, administer medications, and ensure a safe, supportive environment while maintaining detailed documentation and collaborating with physicians and an interdisciplinary group.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse licensed in good standing to practice.
• 1 year of nursing experience in the last 3 years, or BSN with 2 years’ experience in public health, home care, or hospice preferred.
• Management experience not required; may supervise hospice aides.
• Background checks and references required after offer.
• Excellent observation, verbal and written communication skills, problem solving, basic math skills.
• Must be a licensed driver with insured vehicle and CPR certification.

Responsibilities

• Complete initial and ongoing comprehensive assessment of patient and family to determine hospice needs.
• Provide professional nursing care using all elements of the nursing process.
• Develop and revise the plan of care with patient and family involvement.
• Administer medications and treatments as prescribed by the physician.
• Counsel patient and family in meeting nursing and related needs.
• Provide health care instructions and educate on self-care as appropriate.
• Maintain safe patient environment and coordinate care with the interdisciplinary team.
• Participate in on-call duties and supervise hospice aides when required.
