Job summary

Affinity Hospice Management LLC is seeking a hospice aide to provide patient care under RN supervision in Bremerton, WA. You will support personal care, mobility, and daily activities while ensuring safety and comfort for the terminally ill.

The role requires CNA training, NAC, CPR, and a driver’s license. Prior hospice experience is preferred; EMR documentation is required and confidentiality must be maintained.

Qualifications

• Completion of a state approved certified nursing assistant course or equivalent training.
• Nursing Assistant Certified (NAC).
• Current Healthcare Provider CPR certification.
• Current Food Handlers Permit.
• High school diploma or GED.
• Ability to read and follow written instructions and document care given.
• Understands hospice philosophy and provides terminally ill care.
• Prior hospice/home health/long-term care experience preferred.
• Basic computer skills to document in EMR.
• At least 18 years of age.
• Licensed driver with insured vehicle in good condition.

Responsibilities

• Perform personal care needs after assignment and respond to requests promptly.
• Provide positive communication with patient and family within ethical care.
• Meet safety needs and use equipment properly.
• Give personal care including baths, back rubs, oral hygiene, shampoos, bed linen changes.
• Assist in dressing and undressing patients as assigned.
• Assist in feeding, avoiding homemade foods from the caregiver.
• Care for skin to prevent tissue breakdown.
• Report pain or changes in patient condition to RN Case Manager.
• Assist in ambulation and exercise as instructed by nurse or therapist.
• Provide ROM and transfers/positioning as directed.
• Assist with self-administered medications as allowed by state rules.
• Offer bedpans and urinals; light laundry and light homemaker tasks.
• Perform ROM and simple procedures with demonstrated skills competency.
• Provide respite for patient/family/caregiver when on-site.
• Keep living area clean and orderly.
• Adhere to documentation and care procedures.
• Participate in quality assessment teams.
