Job summary

Affinity Hospice Management LLC is seeking a compassionate hospice aide to support patient care under RN supervision. You will perform personal care, assist with mobility and daily activities, and document observations in EMR while maintaining confidentiality and safety.

Qualified candidates have CNA certification, CPR, a high school diploma or GED, and a valid driver’s license. COVID-19 vaccination per policy is required. Prior hospice or long-term care experience is preferred.

Qualifications

• Completion of a state approved certified nursing assistant course or equivalent training.
• Current Healthcare Provider CPR certification.
• COVID-19 vaccination as per facility guidelines.
• High school diploma or GED.
• Ability to read and follow written instructions and document care.
• Understands hospice philosophy and care for terminally ill.
• Prior hospice/long-term care experience preferred.
• Basic computer skills to document in EMR.
• Must be 18 years of age or older.
• Licensed driver with insured vehicle in good working order.

Responsibilities

• Perform personal care tasks according to patient plan and report needs promptly.
• Communicate with patients and families with confidentiality and respect.
• Ensure safety and proper use of equipment (foot stools, side rails, O2).
• Assist with bathing, grooming, oral hygiene, and linen changes.
• Help with dressing, feeding, and mobility as instructed.
• Notice symptoms, report pain or changes to RN Case Manager.
• Assist with range of motion and transfers as directed.
• Support with self-administered meds as allowed by state rules.
• Assist with light housekeeping and laundry and waste disposal.
• Maintain clean, orderly living area and adhere to procedures.
