Job summary

St. Croix Hospice is seeking a Hospice Aide (CNA) to join our team in delivering compassionate end-of-life care in patients’ homes. Under RN supervision, you will provide direct personal care, assist with mobility and daily activities, and document care in the Plan of Care while ensuring comfort and quality of life.

The role emphasizes patient comfort, family support, and adherence to HIPAA and state regulations. A CNA license and reliable transportation are required for this in-home position.

Qualifications

• Active CNA license in the state of practice.
• Must be at least 18 years old.
• Ability to pass background study, consumer report and drug test.
• Valid State Driver’s License and access to a reliable vehicle.

Responsibilities

• Provide personal care including bathing, dressing, oral hygiene, and linen changes.
• Assist with mobility, transfers, and exercises as directed by nursing staff.
• Support meals, feeding, and light household tasks as assigned.
• Monitor patient condition changes and report to the RN Case Manager.
• Maintain clean, safe, and organized patient environment with HIPAA-compliant documentation.
• Provide emotional support and respite for family caregivers.
