Job summary

St. Croix Hospice is seeking a compassionate Hospice Aide (CNA) to join our team delivering in-home end-of-life care. You will work under RN supervision to help patients with daily living activities, comfort measures, and dignity.

Responsibilities include personal care, mobility assistance, meal support, and maintaining accurate documentation per HIPAA and Plan of Care. A valid CNA license in Wisconsin, 18+ years, and a valid driver's license are required.

Qualifications

• Active Certified Nursing Assistant license in the state of practice.
• Minimum age 18.
• Valid driver's license and reliable vehicle.
• Ability to pass background check and drug screen.
• Excellent communication and compassion in home care.

Responsibilities

• Provide direct personal care including bathing, dressing, oral hygiene, and bed linen changes.
• Assist with mobility, transfers, and exercises as directed.
• Support meals, feeding, and light household tasks.
• Monitor patient condition changes and report to RN Case Manager.
• Maintain safe, clean home environment and comply with HIPAA.
• Provide emotional support to patients and respite for families.
• Follow Plan of Care and documentation requirements.
• Use equipment safely and as needed.
