Job summary

AccentCare, Inc. is seeking a dedicated Aide / CNA IPC for Hospice at Franklin Square Hospital. The role is full-time, on-site, with a schedule from 7:00 AM to 7:30 PM and every other weekend.

You will provide compassionate hospice assistance, assist with personal care, mobility, and documentation, and collaborate with the RN and care team to support end-of-life goals. Qualified candidates are state-certified CNAs with CPR, listed on the state aide registry, and able to work in home or facility

Qualifications

• State-certified nurse aide with required training and passing exam
• CPR certification required for direct patient care in relevant states
• Must be listed on state aide registry with clear record and eligible to provide care
• Must have reliable vehicle, valid driver’s license, and auto insurance

Responsibilities

• Provide compassionate, hands-on hospice care including personal hygiene, repositioning, and mobility support
• Observe and report changes in patient condition to RN and interdisciplinary team
• Support patients at home or facilities with movement and activities for dignified end-of-life care
• Document care accurately reflecting the plan of care and goals
• Uphold infection control and safety protocols in home and facility environments
