Job summary

Chapters Health System in Fort Myers, FL, is seeking a Home Care Services Coordinator to develop and implement homecare programs for frail elderly participants.

You will assess homecare needs, coordinate durable medical equipment and services, and collaborate with the IDT to ensure high-quality care.

A valid Florida driver's license, current auto insurance, active BLS, and Alzheimer's training are required; excellent communication and scheduling skills are essential.

Qualifications

• Alzheimer's Disease and Related Dementias training certification required (Florida).
• Education: High school diploma or GED; prefer experience with frail/elderly populations.
• Active BLS certification for healthcare professionals (American Heart Association or Red Cross).

Responsibilities

• Develop and implement homecare services for program participants.
• Assess homecare needs and develop care plans for frail elderly populations.
• Collaborate with other disciplines to ensure quality services meet participant needs.
• Coordinate and authorize durable medical equipment and home services.
• Perform data entry for patient care, demographics, timesheets, and reports; update records timely.
• Communicate with IDT members and participate in interdisciplinary team meetings.
