Job summary

Chapters Health System is seeking an experienced RN Case Manager to assess patient/family needs, coordinate the Plan of Care with the Interdisciplinary Team, and provide clinical, palliative and supportive care in the home setting to keep participants at home as long as possible.

The role requires Florida RN licensure and at least 1 year in hospice or hospital settings, with EMR/EHR experience and a valid driver's license.

Qualifications

• Current RN license in the state of employment.
• Minimum 1 year nursing experience in hospice or hospital setting preferred.
• Experience with EMR/EHR systems required.
• Must have valid driver’s license and automobile insurance.
• Ability to travel to patient homes and perform assessments.
• Excellent written and verbal English communication skills.
• Must be able to lift patients/equipment and meet physical requirements.

Responsibilities

• Provide direct patient/family care as part of the IDT.
• Coordinate Plan of Care reflecting patient evolving needs.
• Educate patients and families about disease, palliative care and safety.
• Communicate with physicians and IDT using SBAR.
• Perform nursing interventions to alleviate symptoms and maximize comfort.
• Document encounters in EMR per policy and participate in IDT meetings.
• Visit patients at home with appropriate frequency.
• Support referrals to service lines and engage with hospitals and facilities.
