Job summary

ABL Health Care LLC in Gilroy is seeking a full-time Registered Nurse (RN) for our Home Health team. The role evaluates patients, creates and implements care plans, and tracks progress in the field with support from the Clinical Director.

Candidates must hold a California RN license, have home health experience, a valid CA driver’s license, and be able to travel. We offer competitive benefits, 401(k), and paid time off.

Qualifications

• Current California RN license required.
• Experience in home health or hospice care preferred.
• Ability to travel as needed.

Responsibilities

• Provide nursing care within scope of practice per individualized care plan.
• Administer treatments, monitor patient conditions, and report changes to the nursing team.
• Maintain accurate clinical records using company devices and systems.
• Supervise LVNs when assigned.
• Adhere to all policies to ensure high standards of care.
