Job summary

ABL Health Care in Antioch is seeking a compassionate, experienced Registered Nurse (RN) to join our Home Health team on a full-time basis. You will assess patients, develop and implement care plans, and coordinate with physicians, case managers, and family members.

This role requires California RN license, Home Health or Hospice experience, and ability to travel within the region. We support you with a back-office team and provide tools to streamline documentation.

Qualifications

• California RN license required.
• Experience in Home Health or Hospice care.
• Valid California Driver's License.
• Willingness to travel within the region.

Responsibilities

• Provide nursing care within the scope of practice per individualized care plan.
• Administer treatments, monitor patient conditions, and report changes to care team.
• Maintain accurate clinical records using company devices and documentation standards.
• Supervise LVNs when assigned.
• Comply with company policies to uphold high standards of care.
