Job summary

ABL Health Care is seeking a compassionate, organized RN for Full-Time Home Health in Danville, CA. The RN will evaluate patients, create and implement care plans, and coordinate with Clinical Directors while providing in-field and remote support.

The role requires a CA RN license, Home Health or Hospice experience, a CA driver's license, and ability to travel. Benefits include health, dental, vision, 401(k), and PTO.

Qualifications

• California RN license required.
• Experience in home health or hospice care.
• CA driver’s license and ability to travel.

Responsibilities

• Provide nursing care within the scope of practice aligned with the patient care plan and Interdisciplinary Team.
• Administer treatments, monitor patients, and report changes to the Physician or Case Manager.
• Maintain accurate clinical records and document using a company-provided device.
• Supervise LVNs when assigned.
• Adhere to all company policies to ensure high standards of care.
