Job summary

ABL Home Health in Lafayette, California is seeking a compassionate, organized Registered Nurse (RN) for full-time field-based home health care. The RN will assess patient needs, implement and monitor care plans, and coordinate with the Clinical Director and team to ensure quality care in the field.

The role requires an active California RN license, home health or hospice experience, a valid CA driver's license, and willingness to travel within the region.

Qualifications

• Current Registered Nurse (RN) License to practice in California – Required.
• Experience in Home Health or Hospice care
• Current CA Driver's License
• Able to travel

Responsibilities

• Provide nursing care within the scope of practice, following the patient's individualized care plan.
• Administer treatments, monitor patient conditions, and report signs/symptoms signaling a change to appropriate personnel.
• Maintain accurate clinical records and submit documentation using a company-provided device.
• Supervise Licensed Vocational Nurses (LVNs) when assigned.
• Comply with all company policies and procedures to ensure high standards of care
