Job summary

ABL Health Care in San Leandro seeks a dedicated full-time Registered Nurse (RN) for our Home Health team. You will assess patients, develop and implement care plans, and monitor progress in the field, collaborating with the Clinical Director and staff.

Ideal candidates hold a CA RN license, have home health or hospice experience, and can travel as needed. We offer comprehensive benefits and a supportive, team-focused environment.

Qualifications

• Current California RN license required.
• Experience in Home Health or Hospice care.
• Current CA Driver's License; able to travel.

Responsibilities

• Provide nursing care within the scope of practice, following the patient's individualized care plan, in coordination with the Interdisciplinary Team.
• Administer treatments, monitor patient conditions, and report changes to Physician, Case Manager, and Director of Nursing as needed.
• Maintain accurate clinical records and submit documentation using a company-provided device.
• Supervise Licensed Vocational Nurses (LVN) when assigned.
• Comply with all company policies and procedures to ensure high standards of care.
