Job summary

PARS Therapy is seeking a compassionate Physical Therapist to join our home health team in Kingwood, TX. You will evaluate patients in their homes, develop personalized treatment plans, and provide interventions to restore mobility and reduce pain while educating patients and caregivers.

Qualifications

• Active Physical Therapist license in Texas
• Professional liability insurance coverage specific to Texas PT license
• Graduate degree or higher in Physical Therapy from an accredited program
• Experience in home health or rehabilitative settings preferred
• Ability to adapt care approaches to diverse patient needs
• Up-to-date knowledge of evidence-based treatment techniques and best practices
• Excellent communication skills and a friendly, professional demeanor with patients

Responsibilities

• Evaluate patients’ physical abilities and functional limitations in a home health setting
• Develop and implement individualized treatment plans to restore function and mobility
• Provide therapeutic interventions including exercise, gait training, manual therapy, and balance retraining
• Educate patients and caregivers on home exercise programs, safety techniques, and adaptive strategies
• Monitor patient progress and adjust plans to achieve optimal outcomes
• Accurately document evaluations, treatment sessions, and progress notes using digital platforms
• Collaborate with physicians, nurses, and occupational therapists
• Conduct home safety assessments and recommend fall-prevention strategies
• Deliver compassionate care while promoting patient independence
