Job summary

PARS Therapy in Katy, TX is seeking a compassionate Physical Therapist to join our home health team. The PT will assess and treat patients in their homes, develop personalized plans, and educate patients and caregivers on safe exercises.

The role emphasizes evidence-based care, collaboration with an interdisciplinary team, and flexible scheduling to support patient independence and improved quality of life.

Qualifications

• Active Texas Physical Therapist license required.
• Professional liability insurance coverage required.
• Graduate degree or higher in Physical Therapy from an accredited program.
• Experience in home health or rehabilitative settings preferred.
• Ability to adapt care approaches to individual patient needs.
• Strong communication skills and collaboration with care teams.
• Up-to-date knowledge of current PT practice.

Responsibilities

• Evaluate patients’ physical abilities and functional limitations in a home health setting.
• Develop and implement individualized treatment plans to restore function, mobility, and reduce pain.
• Provide therapeutic interventions including exercise, gait training, manual therapy, and balance retraining.
• Educate patients and caregivers on home exercise programs, safety techniques, and adaptive strategies.
• Monitor patient progress and adjust plans to achieve optimal outcomes.
• Document evaluations, treatment sessions, and progress notes using digital platforms.
• Collaborate with interdisciplinary team members including physicians, nurses, and occupational therapists.
• Conduct home safety assessments and make recommendations to prevent falls or re-injury.
• Deliver compassionate care while empowering patients to achieve greater independence.
