Job summary

PARS Therapy is seeking a compassionate Physical Therapist to join our home health team in Longview, TX. You will assess patients in their homes, design personalized treatment plans, and help restore mobility and independence.

The role emphasizes patient education, collaboration with an interdisciplinary team, safe home exercise programs, and documentation. A TX license and liability coverage are required, with experience in home health preferred.

Qualifications

• Active Texas Physical Therapist license and liability insurance.
• Graduate degree or higher in Physical Therapy from an accredited program.
• Experience in home health or rehabilitative settings preferred.
• Excellent communication and patient education skills.

Responsibilities

• Evaluate patients’ physical abilities in a home health setting.
• Develop and implement individualized treatment plans to restore function and mobility.
• Provide interventions including exercise, gait training, manual therapy, and balance retraining.
• Educate patients and caregivers on home exercise programs and safety techniques.
• Monitor progress and adjust plans to achieve optimal outcomes.
• Document evaluations, sessions, and progress notes using digital platforms.
• Collaborate with physicians, nurses, and other therapists.
• Conduct home safety assessments and reduce fall risk.
• Deliver compassionate care to empower greater independence.
