Talin Home Healthcare LLC in Sterling, VA is seeking compassionate Home Health Nurses to provide in-home care across the Northern Virginia region. This role offers independence, meaningful patient relationships, and the chance to deliver high-quality clinical care in patients' homes.

As an RN, you will perform assessments, develop care plans, administer medications, manage wound care, IV therapy, and education for patients and families while coordinating with physicians and the care team.

Qualifications

• Active RN license in the state of Virginia.
• Previous home health, hospital med-surg, or community nursing experience preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• Reliable transportation and valid driver's license for travel throughout the NOVA region.
• Ability to work independently and manage a flexible patient caseload.
• Must commit to working a minimum of 2 days a week 1 day of the days must be a Monday, and the other can be Wednesday or Friday.

Responsabilités

• Provide skilled nursing care to patients in their homes across the Northern Virginia area.
• Conduct comprehensive patient assessments and develop individualized care plans.
• Must have experience with administering medications, wound care wet and dry, wound vac, IV therapy, ostomy care and other clinical treatments as ordered.
• Monitor and document patient progress, vital signs, and changes in condition.
• Educate patients and family members on disease management, medications, and home care techniques.
• Collaborate with physicians, therapists, and care team members to ensure coordinated patient care.
• Maintain accurate and timely clinical documentation in compliance with regulatory standards.
