Job summary

The Pennant Group is seeking a Home Health Aide to support Symbii Home Health clients in the Rock Springs area. You will provide essential personal care, assist with activities of daily living, and help maintain a safe and comfortable home environment under the supervision of nursing staff.

You will need to follow care plans, document care provided, and communicate changes in patients' conditions. CPR certification and a valid driver's license are required; the role emphasizes compassion,

Qualifications

Meets state and federal training requirements.
At least 18 years of age.
Ability to read and follow written instructions and document care given.
Self-directing with the ability to work with little direct supervision.
Empathy for the needs of the ill, injured, frail and the impaired.
Possess and maintains CPR certification.
Licensed driver with automobile that is insured.
Home health aide will not perform nursing procedures.

Responsibilities

Provide personal care including baths, oral hygiene, shampoos, and dressing assistance.
Assist in feeding and meal planning as needed.
Take and record basic temperatures, pulse, and blood pressure when ordered.
Assist in ambulation and mobility according to the care plan.
Report changes in patient condition to the assigned nurse and document accordingly.
