Job summary

Fallon Health is seeking a Health Aide/CNA to join Summit Eldercare, delivering essential personal care in home and PACE settings. You’ll work under RN supervision to support ADLs, maintaining a safe, clean environment and contributing to care plans with an interdisciplinary team.

The role emphasizes infection control, documentation, and collaboration with therapists and other staff to ensure high-quality, patient-centered care for older adults in a community-based program.

Qualifications

• Completion of a Home Health Aide or CNA program is highly desirable.
• Certification as HHA or CNA is preferred; reliable transportation required.
• 1 year of experience with frail/older adults or Summit Eldercare training may be required.

Responsibilities

• Assist ADLs for participants at the PACE center and in homes under RN supervision.
• Contribute to care plan development with the Interdisciplinary Team.
• Maintain infection control and cleaning per CDC guidelines.
• Document accurately in EMR and other systems per policy.
• Support meal prep and assist with program activities and transportation as needed.
