Job summary

Fallon Health is hiring Health Aides and CNAs to provide personal care, light housekeeping, and ADL support for Summit Eldercare participants at the PACE center and in homes. Under RN supervision, you’ll help with daily activities, documentation, and infection control in a high-quality, low-volume care setting.

This role emphasizes autonomy, flexibility, and member-centered care within Fallon Health’s Summit Eldercare program. A Driver’s license and reliable transportation are required.

Qualifications

• One year of experience with frail/older adults preferred in a PACE-like setting.
• Certification as Home Health Aide or Nursing Assistant desired.
• Must have valid driver's license, reliable transportation and clean driving record.

Responsibilities

• Assist with Activities of Daily Living at the PACE Center and in participants' homes.
• Contribute to care plan development with Interdisciplinary Team.
• Document accurately in EMR and maintain infection control precautions.
• Support activities staff with individual and group programs and with therapy-related tasks.
