Job summary

The Lakes Home Care, Inc. is seeking a compassionate Home Health Aide to assist patients with daily living and personal care, in patients' homes, following an established care plan.

Responsibilities include helping with baths, ADLs, documenting care, recording vitals, and ensuring home safety. A valid CPR certification and Maxim screenings are required, with a flexible, part-time schedule. The company emphasizes quality care and patient comfort.

Qualifications

• Meets all training requirements in accordance with state and federal laws when applicable
• Ability to read, follow written instructions and document care given
• Possess and maintains current CPR certification in accordance with state regulation
• PPD or chest x-ray if applicable
• Successful completion of Maxim screening examinations
• Participation and completion of Maxim’s competency program

Responsibilities

• Assist patients with activities of daily living according to the plan of care, including baths, oral hygiene, dressing, toileting, feeding
• Take and record vital signs as ordered and demonstrate competency
• Support safe transfers and ambulation
• Provide exercises per care plan and perform ROM as ordered
• Handle patient laundry and personal care tasks
• Observe safety, use equipment properly, and educate on home safety
• Communicate clearly with client, family, and office; document changes to nurse supervisor
