Job summary

The Lakes Home Care Services is seeking a compassionate Home Health Aide to provide daily living assistance and personal care in patients' homes per an established care plan.

Responsibilities include assisting with activities of daily living, documenting care, and communicating with clients and the supervising nurse. Training and CPR certification are required; Maxim competency program completion is expected.

Qualifications

• Meets all training requirements in accordance with state and federal laws when applicable.
• Ability to read, follow written instructions and document care given.
• Possess and maintains current CPR certification in accordance with state regulation.
• Successful completion of Maxim competency program (if applicable).

Responsibilities

• Assists patients with plan of care including baths, hygiene, dressing, toileting, feeding.
• Take and record vital signs when ordered.
• Safe transfer and ambulation; assist with exercises as prescribed.
• Perform ROM and simple procedures as ordered.
• Handle patient laundry and ensure safety at home.
• Observe and document patient condition; report changes to nurse supervisor.
