Job summary

The Lakes Home Care Services in Florida is seeking a compassionate Home Health Aide to support patients at home under an established care plan. You will assist with daily living activities, personal care, and comfort measures according to each patient’s care plan.

Qualifications

• Meets all training requirements in accordance with state and federal laws when applicable
• Ability to read, follow written instructions and document care given
• Possess and maintains current CPR certification in accordance with state regulation
• PPD or chest x-ray if applicable
• Successful completion of Maxim screening examinations
• Participation and completion of Maxim’s competency program
• Must meet all federal, state, and local requirements.

Responsibilities

• Assist patients with activities per plan of care (baths, hygiene, dressing, toileting, feeding).
• Record vitals when ordered and demonstrate competency.
• Support safe transfers and ambulation.
• Provide exercises according to the plan of care.
• Assist with ROM and extension therapy as ordered.
• Handle patient laundry and maintain safety with equipment.
• Observe, document patient condition, and report changes to nurse.
• Communicate clearly with client, family, and office.
• Follow the plan of care and complete assigned tasks.
• Observe home safety and emergency procedures.
