Job summary

UnitedHealth Group, via Heart N Home Hospice (a part of LHC Group within Optum), invites you to join a caring team dedicated to helping people stay safely at home. As a Home and Community Based Services Aide, you'll provide personal care and assist with daily living activities.

Responsibilities include bathing, grooming, meal prep, documenting client condition with EVV, ensuring safety, and supporting clients in diverse situations.

Qualifications

• Must have valid driver's license, vehicle insurance, and reliable transportation.
• CPR certification preferred or able to obtain within 90 days.
• 6+ months of home care experience.
• Ability to work flexible hours and independently.

Responsibilities

• Provide personal care and assist with daily living activities (bathing, grooming, dressing, ambulation, medication reminders).
• Support household tasks, meal preparation, and accompany clients to appointments or errands.
• Monitor and document client condition, vital signs, and incidents using EVV system.
• Ensure a safe environment and respond promptly to client needs.
