Job summary

Lakeview Hospital in Minnesota is seeking a Home Health Aide to join our Homecare/Hospice team. You will assist professional staff in delivering patient care, support daily living activities, and help ensure safe home environments.

The role is 0.6 FTE with a 8:00–16:30 schedule, three days per week; travel to clients' residences is required; candidates must have a valid driver’s license, MN or WI CNA certification, and BLS CPR.

Qualifications

• Education: high school diploma or equivalent.
• CNA certificate MN or WI required at start date.
• Current BLS CPR/AED required.

Responsibilities

• Assist professional staff to provide patient care.
• Take vital signs and record I&O.
• Support ADLs and safe home environment.
• Drive to clients' residences and document care in EHR.
• Observe patient condition and report changes to Case Manager.
• Perform routine tasks to maintain department operations.
