Job summary

The Lakes Home Care, Inc. is seeking a compassionate Home Health Aide to provide daily living and personal care services in patients' homes according to an established care plan.

You will assist with bathing, dressing, grooming, meals, mobility, and safety in the home environment. Responsibilities include monitoring patients, recording vital signs when ordered, communicating with clients and supervisors, and adhering to plan of care.

Qualifications

• Meets all training requirements in accordance with state and federal laws.
• Ability to read, follow written instructions and document care given.
• CPR certification current as required by state regulation.
• Maxim competency program completion as specified

Responsibilities

• Assist patients with daily living tasks as per care plan (baths, hygiene, dressing, feeding).
• Monitor vital signs and report as ordered; document results.
• Ensure safe transfers and ambulation according to care plan.
• Support exercise and ROM as directed by the plan of care.
• Handle patient laundry and maintain safety with equipment.
• Observe and report changes to supervising nurse.
